Summary:

Mount Evans is seeking a Full Time LPN Monday-Friday for Home Health and Hospice care visits. The Licensed/Practical Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with members of the team to help meet positive patient care outcomes.

Essential Functions:

· This position provides direct patient care as defined in State Nurse Practice Act, including specialized duties related to end of life care.

· Implements plan of care initiated by the registered nurse.

· Provides accurate and timely documentation consistent with the plan of care.

· Assesses and provides patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.

· Participates in coordination of home health or hospice services, appropriately reporting the identified needs to the interdisciplinary team members, to the registered nurse and/or clinical manager.

· Uses and prepares equipment and supplies effectively, adhering to aseptic technique.

· Provides appropriate pain/symptom management. Evaluates patient’s response to treatments/medications.

· Participates in personal, professional growth and development.

· Performs other duties as required and necessary to ensure the success of the Agency.

Supervisory duties:

· None.

Knowledge, skills and abilities:

· Ability to consistently promote, support, work, and act in a manner in support of the Mount Evans mission and values.

· General knowledge of nursing and health care terminology.

· Excellent patient care skills.

· Strong knowledge of and compliance with accepted professional standards and practices.

· Demonstrates good verbal and written communication, and organizational skills.

· Understands philosophy of hospice concept. Also understands the needs of the terminally ill.

Education:

· Graduate of an accredited practical nurse or vocational nursing program.

· Currently licensed as an LPN in the state.

· Possess and maintain CPR certification.

Experience:

· Three (3) years nursing experience is required. Home health, hospice or medical/surgical experience is preferred.

· Must be able to lift up to 50 pounds at a time.

Job Type: Full-time

Pay: $30.00 - $35.00 per hour
